We rely on people like you for contributions/pledges in order to support our efforts. If you can donate or pledge any amount it would be appreciated. As a citizen based organization we depend upon your support and help. Note: we are a 501c4 non-profit legislative advocacy organization and therefore NOT tax deductible.

METHODS OF DONATING:

1- Send check or money order to above address.

2- If you wish to make a donation or monthly pledge by credit card, send an email to ginny@citizenshealth.org and we will send/fax a form to fill out.  Or print off form from AUTOMATIC BILLING AUTHORIZATION FORM and mail to us.  We can process American Express, Visa, Discover and MasterCard. 

3- If you wish to have a monthly pledge automatically deducted from your checking account, complete the CC4HF portion of this  Automatic Contribution System form (ACS) and send the ACS form and a voided check to the above address.  (In order to open the PDF format you need a Acrobat Reader.  You can download a free viewer from Adobe.)
